Welcome New Patients!
Your visit to our practice, Maple Springs Foot Center, establishes a vital foundation for our relationship with you. During the first visit we will make sure to obtain important background information, your medical history, and give you time to get to know Dr. Snyder.
When filling out the “new patient paperwork” please be specific with your medications. Include the name and dosage for each medicine. If providing a list is more convenient for you, please do so. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

What you should bring...
Please bring your insurance card(s) with you to your appointment. If you are providing us with a copy of the insurance card(s), please have the front and back copied for our records.

We do our best to have patients seen at the time of their appointment, in order for us to continue to do so, please be on time for your appointment. If you choose to fill out the new patient paperwork the day of your visit, please arrive 5-10 minutes prior to your appointment time.
